The applicant/legal guardian must fill out the Application for Disability Access Parking Privileges (Form MVR 32-6-230). The application is available at a local DMV or printed online.
Disability access placards are issued at no cost.
Applicants will need to reapply by completing a Recertification for Handicapped Parking Privileges. If disabled individuals qualify for long-term disability after their first application, they may recertify by signing the certification at the bottom of the form.
While permanent placards are valid for 5 years, temporary placards are valid for a period of no more than 6 months.
Individuals who wish to obtain a duplicate placard must complete and sign the application for replacement disability access license plate and/or placard. The form can then be submitted to the county licensing office.
The placard must be hung from the rearview mirror located on the front windshield when the vehicle is parked in the designated parking space. When there is no rearview mirror, the placard must be displayed on the dashboard.
Drivers are required to remove their permit from sight when not parking.
The dates for issuing licenses is based on the first letter of your last name. For example, if your last name begins with an “F” or an “N,” you would need to renew during the month of April.
